The leak was seen by users placing an order for the standard edition of the game (not the deluxe edition) using their PlayStation 5, as Golem said in his report. It is stated that once someone would try to schedule the pre-loading process after doing the pre-order, the image would be shown as compared to the purchased page with the adult artwork.

Dusk Golem mentioned that they checked the leak with contacts to absolutely confirm it. The source explained that this was a genuine accident by PlayStation.
